Confused by the range of supplements on the market? Walk into any grocery store or big-box retailer and you’ll find an overwhelming wall of supplements. There are hundreds of brands all making similar claims (at wildly different price points), yet most people do not realize that the supplement industry is not regulated using the same standards as prescription medications.
This fact means that quality, potency, purity, and even the form of the ingredient used can vary enormously from one brand to the next. For instance, two bottles labeled “Vitamin D 5,000 IU” can perform very differently in your body depending on how they were made, what they contain, and whether what’s listed on the label is actually in the bottle.
5 Supplements Our Pharmacists Trust
The role of a compounding pharmacy includes helping patients understand their treatment options. At Eli Pharmacy, our pharmacists have done the work of vetting supplement brands so you don’t have to. We look at third-party testing certifications, the specific forms of each nutrient used, bioavailability research, ingredient sourcing and manufacturing standards. Here are the five supplements we recommend to nearly every patient, along with information about what makes a high-quality version worth choosing over a cheap alternative.
1. A Quality Multivitamin – Your Nutritional Foundation
Even with a well-balanced diet, most people have nutritional gaps. Depleted soil, processed foods, busy lifestyles and individual differences in absorption means that getting everything you need from food alone is harder than it sounds. A high-quality multivitamin consistently fills those gaps.
What separates a good multivitamin from a poor one comes down to the forms of the nutrients included. Many budget brands rely on cheap or poorly absorbed forms, like folic acid instead of methylfolate, or cyanocobalamin instead of methylcobalamin (B12).
For people carrying the MTHFR gene variant (more common than most people realize), synthetic folic acid cannot be properly converted and may actually accumulate in harmful amounts. A quality multivitamin uses activated, bioavailable forms of each nutrient; for example, methylated B vitamins, chelated minerals, and natural rather than synthetic vitamin E.
A better multivitamin should include methylated B vitamins, chelated minerals, no unnecessary fillers or artificial colors, and utilize third-party testing.
2. Vitamin D3 With K2 – The Combination Your Bones and Heart Need
Vitamin D deficiency is one of the most widespread nutritional deficiencies in the United States, affecting an estimated 40% of adults. It plays a critical role in immune function, mood regulation, bone density, muscle strength and cardiovascular health. Most people are aware they should be taking vitamin D, but far fewer know why it should always be paired with vitamin K2.
It is well-known that vitamin D increases calcium absorption from food and supplements, and that’s exactly what you want for bone health. But without K2 that calcium has no clear direction, and it can be deposited in soft tissues and arterial walls rather than in bones, where it belongs.
Vitamin K2 (specifically the MK-7 form, which has the longest half-life in the body) activates proteins that direct calcium into bones and away from arteries. Understand that taking vitamin D without K2, especially at higher doses, may actually increase cardiovascular risk over time.
For proper absorption and support, choose vitamin D3, also called cholecalciferol, paired with K2 as MK-7 in an oil-based softgel.
3. A Targeted Probiotic – Supporting Your Gut, Immunity and Beyond
The research on the gut microbiome has exploded over the past decade, and the findings are remarkable. The trillions of bacteria residing in your digestive tract influence not just digestion, but also immune function, skin health, weight regulation, cardiovascular risk and even mental health.
Unfortunately, a proper microbial balance in the gut can easily be disrupted by antibiotic use, stress, poor diet or illness. However, using a quality probiotic helps restore and maintain the desired healthy microbial balance.
This is a category where brand quality varies enormously. By the time they reach you, many shelf-stable probiotics will have lost significant viability. And strain specificity matters as well, as not all Lactobacillus strains accomplish the same results. Note that the research on probiotic benefits is tied to specific strains at specific doses.
A bottle claiming “10 billion CFU” means little if the strains are not clinically researched, and if the product was not shipped and stored correctly.
When choosing a probiotic pay close attention to clinically studied strains, guaranteed potency at expiration, proper storage, and formulas that match your health goals.
4. Omega-3 Fatty Acids – Anti-Inflammatory Support From the Ground Up
Omega-3 fatty acids, specifically EPA and DHA, are among the most extensively researched nutrients in medicine. The evidence for their benefits spans cardiovascular health, brain function, joint inflammation, eye health, mood, and pregnancy outcomes.
Most Americans consume far too few omega-3s relative to the pro-inflammatory omega-6 fatty acids that dominate the modern diet. Supplementing with a quality fish oil or algae-based omega-3 helps restore a healthier ratio.
The problem with inexpensive fish oil is threefold: rancidity, inadequate EPA/DHA content, along with poor bioavailability. Fish oil does oxidize rapidly, and a rancid product will do more harm than good.
Many budget brands also contain far less EPA and DHA than the label implies, once you account for the actual concentration. Essentially, while the serving size might look impressive, the active content is minimal. Understand that the triglyceride form of omega-3 is absorbed significantly better than the ethyl ester form commonly found in lower-cost products.
A quality omega-3 supplement should use the triglyceride form, provide at least 1,000–2,000mg of combined EPA+DHA per serving, and be tested for purity.
5. Magnesium – The Most Underrated Mineral in Your Cabinet
Magnesium is involved in over 300 enzymatic reactions in the body, and yet an estimated 50–60% of Americans fail to get adequate amounts through diet alone. The role of magnesium spans energy production, muscle and nerve function, blood sugar regulation, blood pressure management, protein synthesis and sleep quality.
Many patients who come to us struggling with poor sleep, muscle cramps, anxiety, constipation, or persistent fatigue are unknowingly running low on magnesium.
Plus, magnesium is one of the supplements where form makes an enormous difference. The most common form of magnesium found in drugstore brands is magnesium oxide. Unfortunately, it is cheap and poorly absorbed, with a bioavailability of less than 10%. Magnesium oxide largely acts as a laxative and provides little systemic benefit.
In contrast, magnesium glycinate is highly bioavailable, gentle on digestion, and particularly effective for sleep and anxiety. Magnesium malate supports energy production and is well-tolerated. Magnesium threonate is the only form shown to cross the blood-brain barrier, making it uniquely valuable for cognitive and neurological support.
The right magnesium depends on your needs, so avoid magnesium oxide and choose glycinate for sleep and muscle relaxation, malate for energy or threonate for brain health.
